Impact of home- based physical training programme on values of pulmonary function test and aerobic exercise capacity of children with cystic fibrosis

This diploma thesis takes interest in the issue of physical activities of children with cystic fibrosis, in possible impacts of such activities on clinical condition, in options of prescription of physical activities based on the actual clinical state of a person with cystic fibrosis, and in options of objectification of changes induced by an exercise programme. The theoretic part of this thesis summarizes the current findings about cystic fibrosis, therapy and physical activities in the context of prognosis of patients with cystic fibrosis. In experimental part, we performed examinations on few participants, who were willing to perform a home-based exercise programme. Also, we made a short exploratory questionnaire focused on the attitude of children with cystic fibrosis towards physical exercises. Methodology: We observed the evolution of maximal oxygen uptake VO2max, forced expiratory volume in 1 second (FEV1), forced vital capacity (FVC) and of the scores of 1- minute sit-to-stand test (1-MSTST). All the measurements were performed before and after 12- weeks long exercise intervention. The Assessment of Heart Rate Dynamics and of the QTc Interval During Recovery Phase Depending on the Position of the Body

At present, doctors are not consistent in the way they set the QTc interval in the recovery phase. The main goal of this study was to screen healthy subjects in the two different exercise stress tests and to compare the obtained data from both tests. We wanted to explore whether the heart rate and the QTc interval differs from each other when performed in two different body positions during the recovery phase. The main purpose was to present evidence that would prove or disprove a hypothesis that the figures differ in the different body positions. In the theoretical part we submitted the main information about the heart rate, the QT interval and the relationship between them. We compared the behaviour of the obtained data during the exercise and during the recovery phase. We described the causes of the QTc prolongation and how it is related to the cardiac arrhythmias. In the practical part we examined 20 healthy subjects. Each of them underwent two exercise stress tests on the bicycle ergometer, up to the subjective maximum level of the exercise intensity. Zjišťování maximální tepové frekvence u vybrané populační skupiny zátěžovým testem

The aim of this thesis was to measure maximum heart rate (HRmax) of selected group of volunteers using sport testers. The group consisted of 20 adults (10 women and 10 men) in average age 25.4 SD 2.44 years, average height 171.75 SD 7.99 cm, average weight 69.2 SD 11.08 kg and average BMI 23.33 SD 2.59 (kg/m2). All tested persons were subjected to stress tests on treadmill inSPORTline Gallop (Czech Republic), which tested HRmax (1 km long stress test with 4 ° belt inclination, 1 km long stress test with 6 ° belt inclination, the stress test presented by 1 km run and the graded exercise test). During testing, the heart rate of the runners was measured by the elastic chest band and it was displayed on the SIGMA SPORT RC 14.11 device. Probands ran into Vita Maxima - a state of subjective exhaustion. It was found that for a sample of 20 patients, the selection of the stress protocol did not have statistically significant effect on the values of HRmax. During investigation of the relationship between 10 selected prediction equations for calculating the HRmax, they were found out to be 100% interchangeable. It was found out that the HRmax values obtained by performing stress tests on treadmill and HRmax values calculated using predictive equations for calculating the HRmax differ significantly in terms of statistics. The study confirmed the influence of age to be the factor that affects HRmax the most significantly. The influence of the fitness index and sex on HRmax values was not confirmed in this study.
Comparison of different exercise protocols

The aim of this thesis was to summarize current findings about exercise testing. The main goal was to evaluate protocols used in daily practice, their indications and benefits, comparability and parameters that can influence the results. The purpose of the practical part was to compare the maximal values of physiological parameters (heart rate, VO2, minute ventilation, RER, power output and rated perceived exertion) for the two most commonly used protocols for bicycle ergometry tests in the Czech Republic. We compared ramp (1/3W.kg-1 .min-1 ) and step (1W.kg-1 every 3 min) incremental protocols to exhaustion. Thirteen young, healthy and physically fit subjects (9 males and 4 women aged from 20 to 31 years) underwent two tests in separate occasions, during which they performed the protocols in random order. Paired t-test was used to statisticaly analyze the data. We found no statisticaly significant differences in these maximal values except for the exercise test time. We deduced some practical benefits of each protokol from obtained data.
Aerobic fitness and physical activity in children with primary ciliary dyskinesia

Primary ciliary dyskinesia is a rare hereditary disorder with impairment of cilia characterized by chronic cough with sputum, bronchiectasis or pneumonia. Regular exercise training should affect pulmonary function, promote mucociliary clearance and improve quality of life. The aim of this thesis is to compare the aerobic fitness of patients with PCD with a control group.
